Report Vaccine-Preventable Diseases (VPDs) To report a 4-hour "presumptive" VPD call 1-800-469-4599. To report a 24-hour "presumptive" VPD, call 1-800-469-4599 or complete the Communicable Disease REPORT Card. To report a 5-day Standard VPD complete the Communicable Disease REPORT Card.
